This is a great opportunity to be an integral part of our Assertive Community Treatment Team to provide services to our most vulnerable patients. As the ACT Specialist some of your duties will be to greet and schedule patients, maintain and track a log of ACT patients with conditional discharges while making leadership aware of any conditional discharge renewals that are approaching. You will also attend daily ACT Team meetings, check messages for the ACT office and any additional tasks assigned by ACT leadership.

Qualifications: Bachelor’s degree in a Human Services Field preferred but not required. One to two years of experience with emphasis in mental health preferred, or lived experience.
