**MAJOR RESPONSIBILITIES**:
**To provide case management services to participants experiencing homelessness in the Rapid Rehousing Program.**

**DETAILED RESPONSIBILITIES**:

- Provide support to participants as they transition from the shelter to permanent housing and stability in the community.
- Coordinate and monitor all aspects of service delivery including assessment, recommendations on housing stabilization, treatment planning, budgeting, referrals for adjunctive services, family meetings, group work, individual counseling, advocacy, and liaison with other community agencies.
- Manage all aspects of service delivery, including assessment, Housing stability planning, budgeting help, referrals, group sessions, advocacy, and coordination with other agencies.
- Assist participants in accessing programs like Medicare, WIC, TANF, SNAP, SSI/SSDI to ensure they receive entitled benefits.
- Guide participants in finding housing options, navigating the procurement process, and negotiating with landlords for suitable accommodations.
- Complete ongoing housing reviews/reassessments through regular client contact to appraise changing needs and progress toward goals of maintaining permanent housing.
- Regularly assess participants' needs and progress through home visits, phone calls, office meetings, and community visits.
- Maintain accurate and confidential records and files of participant information, interactions, and progress.
- Enter case notes promptly and accurately into the HMIS tracking system to document client activities and progress.
- Safely transport participants when necessary, ensuring their well-being during transit.
- Participate in case collaboration meetings with supervisors, collaborating agencies, landlords, etc. Attend and actively contribute to case collaboration, supervision, and training meetings.
- Take on other tasks assigned by the Director of Homeless Services or Senior staff as needed.

**REQUIRED QUALIFICATIONS**:

- Excellent verbal and written communication skills
- Strong organizational skills and meticulous attention to detail
- Team Player with good listening skills
- Proficient in the use of Word, Excel and PowerPoint
- ** REQUIRED Valid New York State Driver's License and own insured Vehicle**:

**EDUCATION/TRAINING/EXPERIENCE**:
