Hospice Chaplain

**About the Job**:
We are currently seeking qualified and professional Chaplains to join our Home Healthcare agency. Our Home Healthcare agency services areas across all counties of the Bay Area. We have a current need for Chaplains who are eager and able to work in the Bay Area. We provide a comprehensive care program to each of our patients. To our staff, we offer flexible schedules, A friendly and professional environment, and a true team approach to healthcare.

**Requirements include (but not limited to)**:

- Develop a plan of care for each assigned patient.
- Assesses patient needs, plans for, evaluates, and modifies care to meet goals of spiritual support.
- Collaborates with all disciplines to plan and evaluate team goals for each patient.

We are currently seeking a professional chaplain for home health patient visits. If you are friendly, have a positive attitude, and are genuinely compassionate about helping others, then this is your opportunity to make a difference in the lives of homebound patients and their families.
