**Haven Health, **the leading Home Health, Hospice, and Palliative Care Company in Southern California, is currently hiring an RN Case Manager in the Orange County area to fill our position within our Hospice division.

**Position Available**:
- Full-Time
- Monday - Friday

**Responsibilities**:
- Case manage and direct team on patient care needs
- Provide skilled nursing care to patients in their homes as necessary
- Attend and actively participate in Interdisciplinary Care Teams
**Requirements**:
**Qualifications**:
- Current California RN license
- Minimum of one (1) year of experience
- Excellent written and verbal communication skills
- Car insurance, ability to travel throughout Orange County
