**POSITION GOAL**

Responsible for providing efficient, thorough, and effective case management services which include a comprehensive assessment of clients, development of a written Care Plan with measurable outcomes, arrangement and coordination of services, and client follow-up to monitor the effectiveness of the services arranged and assess the need for additional services.

**PRIMARY RESPONSIBILITIES**
- Provide in-home services employees with the list of clients in need of personal care and housekeeping services. Monitor and review in-home services paperwork and units of services.
- Conduct client intakes using the City of Los Angeles Department of Aging (DOA) assessment with potential clients to identify needs and determine client eligibility. Perform in-home assessment within 14 days of completing intake process.
- Case management responsibilities including the following:
Conduct a comprehensive in-home assessment and write a biopsychosocial assessment; complete paper documentation and input documentation into software system for the City of Los Angeles.

Develop a written care plan with measurable outcomes to address client needs and resolve problems.

Arrange, coordinate and supervise in-home services on behalf of the client.

Maintain contact with client to monitor the effectiveness of the services arranged for the client and assess the need for additional services.

Enter services recording into software system on a weekly basis.

Provide members of the senior community and clients with information referrals, and follow-up to ensure services have been rendered.
- Prepare for and participate in weekly case management staff meetings, and outside training.
- Maintain current A.I.R.S certification.
- Participate in Information & Referral assignments and provide supervision and consultation to I&R specialists and MSW and Bachelor’s field placement students.
- Educate prospective clients about WLCAC Home Care Services, conduct initial intake, home visits and follow-up as required.
- Present information and resources to community groups and professionals.
- Must be able to work agency’s office hours, 8 am to 4:30 pm.

**OTHER RESPONSIBILITIES**

Due to changing program demands, funding source expectations and/or changes in agency policy, as assigned, all staff may be expected to perform other duties, including support of Information & Referral (I&R) Services and benefits enrollment.

**POSITION QUALIFICATIONS**
- Educational Requirements

Master’s degree in human services or related field preferred, will accept Bachelor’s degree in human services or related field.

Minimum two years’ experience working with older adults or at-risk population.

Fluency in English (bilingual in Spanish strongly preferred)

Previous experience working with an ethically and economically diverse population.

Previous care management experience strongly preferred.
- Other Requirements

Must have reliable transportation, current driver’s license, clean driving record, and proof of auto insurance.

Travel is required within the City and County of Los Angeles.

Must pass a criminal background check.

**WORK ENVIRONMENT**

The employee may be expected to enter premises during field visits that may be environmentally challenging; including but not limited to environments with cigarette smoke, bed bugs, dust and other allergens.

Pay: $3,500.00 - $4,000.00 per month
