**Day Center Assistant Case Manager Job Description**

Lifebridge provides intensive Case Management support and services for the men and women we serve. The Day Center Assistant Case Manager position plays a vital role in the success of the organization, as they are responsible for working directly with individuals experiencing homelessness and assisting those individuals in connecting to appropriate services to end their homelessness.

**Qualifications**:

- Minimum of high school diploma or GED
- Familiarity with Microsoft Office (Word, Excel, Outlook)
- Ability to work Monday-Friday, 8:00am-4:00pm, with some on-call hours required.
- Ability to respond safely, quickly, and calmly in emergencies and emergency situations.
- Sensitivity and patience to work with clients suffering from mental illness, alcohol and other substance addiction and abuse.
- Ability to multitask.
- Ability to be resilient in difficult and ambiguous situations.
- Ability to speak (as well as listen) in a non-judgmental manner.
- Valid Massachusetts driver’s license.

**Responsibilities**:

- Assist current case management team with new client intakes.
- Assist clients in obtaining core documents (State ID, MassHealth Card, Social Sec. Cards)
- Assist in referring clients to providers (healthcare, behavioral health, recovery services)
- Client redirection

Pay: From $40,000.00 per year
