We are seeking a skilled and detail-oriented MDS Coordinator to join our team. As an MDS Coordinator, you will be responsible for ensuring accurate and timely completion of the Minimum Data Set (MDS) assessments and coordinating care planning for our residents. MDS Coordinator is a vital role in our organization, as it directly impacts the quality of care provided to our residents.
**Job Qualifications**:
- Understanding of medical terminology
- Previous experience in a nursing home or medical office setting preferred
- Familiarity with clinical documentation improvement principles
- Strong organizational skills with attention to detail
- Ability to effectively communicate and collaborate with interdisciplinary team members
- Knowledge of utilization management principles and practices
- Understanding of HIPAA regulations and ability to maintain confidentiality of resident information
- Excellent problem-solving skills and ability to make sound decisions
**Responsibilities include but are not limited to**:
- Collect and analyze data to determine appropriate care plans for each resident
- Collaborate with interdisciplinary team members to develop individualized care plans
- Ensure compliance with all regulatory requirements related to MDS assessments
- Review medical records and documentation to ensure accuracy and completeness
- Coordinate with healthcare providers to obtain necessary information for assessments and care planning
- Participate in utilization review activities to optimize resource allocation
- Provide education and training to staff on proper clinical documentation practices
- Maintain confidentiality and adhere to HIPAA guidelines in handling resident information
