Pca/caregiver

We are seeking PCA/Caregivers to assist brain injury survivors in achieving their goals and independence.**

As a PCA/Caregiver, you will be providing 1-on-1 assistance with an individual in their home. We strive to provide outstanding services through deep, meaningful relationships between staff and individuals. All services provided are detailed through a personal care plan that is custom-made for every individual.

**Responsibilities**:

- Providing direct support and supervision of assigned individual, with the goal of increasing and achieving quality of life as defined by the individual.
- Establish and maintain effective, professional communication and relationships with individuals, co-workers, family members and other essential team members.
- Provide transportation into the community (grocery shopping, medical appointments, etc.)
- Being aware of individuals medical conditions and providing reminders to take medications.

**Applicants will need to meet the following requirements**:

- Minimum high school diploma or GED required
- Must have a valid driver’s license, automobile insurance and a reliable vehicle.
- Must submit to a criminal background check, and motor vehicle driving record.

**Job Type / Hours**: Part-time: Monday through Friday (9:00am - 2:00pm)
