**JOB TITLE: Case Manager, CHAP (Comprehensive Housing Assistance Program)**

**Basic Functions**:
To provide housing location or relocation support, budget counseling and case management to clients referred to the program. All referrals come from Lancaster General Health, who contracts with Tenfold to provide these services:
**Essential Functions**:
1. Provide mediation and advocacy with landlords on client’s behalf to develop a workable plan to obtain and maintain housing.

2. Assist clients in identifying and utilizing available housing assistance programs.

3. Develop a comprehensive case plan with clients. This shall include detailed short term and long term goals that meet the client’s needs and establish a path toward self sufficiency. Plans are created during the first or second interaction and updated regularly while the client is in the program.

4. Establish and encourage adherence to a personal budget and case management plan through frequent pro-active housing and budget counseling sessions.

5. Create and maintain consistent communication channels, both verbal and written, between several parties i.e. tenant, landlord, debtors and creditors

6. Maintain consistent and frequent communication with referring clinic staff, including attendance at meetings with clinic staff.

7. Provide information and referral assistance regarding available support from appropriate social service agencies and/or community programs.

9. Provide direct case management, home visits and follow up services to each client to ensure long term housing stability, prevention of homelessness, and further progress towards self-sufficiency. Assist participants to create goals for long term self-sufficiency.

10. Maintain accurate and current case notes and files for each client

11. Maintain client confidentiality at all times.

12. Transport clients as deemed necessary within Tenfold’s safety protocols. Transportation requirements should be limited to housing and job searches and occasional visit to relevant social service agencies.

13. Assist in the preparation of any required reporting as instructed by the Program Manager or Program Director

**Other Functions**:
1. Complete data entry in HMIS system

2. Maintain a complete working file providing activity documentation and copies of all corresponding paperwork.

3. Assume other responsibilities as assigned.

**Minimum Knowledge, Skills and Abilities**:

- Minimum Education Requirements: 2-year post-secondary education required. Bachelor’s degree in human services preferred.
- One year of relevant experience required.
- **Ability to speak, read and write English and **Spanish** **required.**
- Excellent communication skills especially listening, mediation and writing skills.
- Strong organizational skills with ability to meet a demanding workload.
- Detail oriented to complete requirements of files and contract compliance.
- Commitment to the Housing First Philosophy
- Basic knowledge of tenant’s rights and responsibilities case management best practices
- Concentrated record keeping, budgeting and mathematical skills.
- Demonstrated knowledge of county social service agencies and community programs
- Experience with computer and proficiency with Microsoft Office suite.
- A commitment to empowering others to solve their own problems.
- A conviction about the capacity of people to grow and change.
- Establish trusting relationships with persons served to help them gain skills and confidence.
- The ability to work collaboratively with other personnel and/or service providers.
- Valid driver’s license and legal vehicle that is registered, inspected and insured.
- Willingness to travel throughout Lancaster city and county.

Pay: $18.00 - $20.00 per hour

Expected hours: 40 per week
