Vienna hospice is currently seeking a Hospice Licensed Vocational Nurse (LVN) to work **PER-DIEM. **We are looking for people who will go above and beyond to make sure that direct patient care under the supervision of a Registered Nurse is top notch. **Responsibilities**:

- Providing skilled nursing patient care under the supervision of a registered nurse.
- Follows the plan of care and follows the comprehensive assessment
- Provides accurate and timely documentation of patient services to reflect on the plan of care
- Administers medication and treatments as ordered by the physician
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
- Reports the ongoing changes about the patient to the interdisciplinary team, includes, registered nurse, hospice aid, and DOPCS.
- Able to perform other duties as assigned by the RN or DOPCS.

**Qualifications**:

- Has a California State license as a Licensed Vocational Nurse
- **A minimum of (1) year experience with Hospice.**:

- **Has experience with Hospice MD**:

- Demonstrates good verbal and written organization skills.
- Must be a California licensed driver with an automobile.
- Has a flexible working hour schedule and be available to work on weekends or continuous care shifts; if needed.
- Understands the philosophy of hospice concept and the needs of terminally ill patients.
