Supervising Care Manager for the Enhanced Care Management Program

**About us**

Health Projects Center is a 501(c)3 non-profit corporation established in 1980. We provide exceptional community-based programs that support elders to live and age with dignity at home. We deliver culturally appropriate programs in both English and Spanish for older adults, family caregivers and health workers.

Health Projects Center serves Monterey, San Benito, and Santa Cruz Counties. We run the Del Mar Caregiver Resource Center, Multipurpose Senior Services Program, and the Central Coast Area Health Education Center. Our programs support over 1,300 families annually to live and age at home at a fraction of the cost of an institution.

**Why This Role is Important to Us**:
This role as Supervising Care Manager for the Enhanced Care Management Program at Health Projects Center is vital to our mission of supporting members of our community. By overseeing and supporting a dedicated team, this position ensures the delivery of high-quality care management services in compliance with regulatory standards. The Supervising Care Manager plays a crucial role in guiding our staff through problem resolution, crisis management, and ongoing professional development. Their leadership in conducting quality assurance activities, fostering community engagement, and representing HPC to external stakeholders is instrumental in our commitment to providing culturally appropriate programs that empower older adults and their families. Moreover, their contributions to program improvement and efficiency drive our continuous efforts to enhance the services we offer.

**Essential Functions**
- Supervision and Team Support:

- Supervises and supports care manager(s), care manager aide(s), and community health worker(s).
- Supports and motivates staff to achieve program objectives.
- Provides timely and constructive feedback on performance to staff.
- Guides the care management team with problem resolution and crisis management.
- Delivers appropriate in-service education and training for care management staff.
- Ensures effective use of technology by care management team.
- Quality Assurance and Compliance:

- Plans and conducts quality assurance activities.
- Ensures compliance with regulations by HPC team.
- Assists with preparing reports for funders and HPC leadership.
- Prepares for and manages audits of services by funders.
- Community Engagement and Representation:

- Coordinates and outreaches with community service providers.
- Manages Community Health Worker Program.
- Represents HPC to outside organizations and potential clients.
- Leadership and Collaboration:

- Serves as a member of the HPC Leadership Team.
- Develops and facilitates effective collaboration and communication between clinical and administrative staff.
- Program Improvement and Efficiency:

- Identifies and implements changes to improve organizational and service efficiency and effectiveness.
- Backup Support:

- This position serves as the backup support for the Director of Care Management.

**Qualifications**:

- Master?s degree in social work, nursing, psychology, counseling, rehabilitation, gerontology, sociology or relevant field.
- Two years of experience working directly with adults.
- Demonstrated excellent oral and written communication skills.
- Demonstrated commitment to program mission.
- Current CA driver?s license and reliable, insured vehicle required.

**Preferred Qualifications**:

- Two years of experience with mental/behavioral health
- Two years supervisory experience.
- Two years case management experience.
- Spanish language fluency.

**Conditions of Employment**
- Possession of a valid California driver's license and access to an insured and reliable vehicle.
- Must pass a criminal background check.
- COVID-19 vaccination required.
- Exempt Status

**Compensation and Benefits**
- Starting salary is $80,000 - $95,000 per hour depending upon skills and experience. We pride ourselves in maintaining a flexible and friendly work environment. We provide comprehensive benefits including: paid vacation (15 days); paid sick leave (80 hours); company paid medical, dental, vision, chiropractor/acupuncture, and life insurance; travel reimbursement; educational/fitness allowance; and pre-tax retirement plan with 6% of salary contributed by employer.
