1. Provides safe, direct physical care of the resident.
2. Maintains a clean, safe and orderly resident environment by keeping the area clean and disposing of soiled, contaminated items.
3. Reports malfunctioning, broken, or hazardous equipment or conditions to the supervisor.
4. Observes policies and procedures as demonstrated through performance of procedure and tasks.
5. Conducts and documents a thorough assessment of each resident’s medical status upon admission and throughout the resident’s course of treatment.
6. Assists in the development and implementation of an individualized treatment plan for each assigned resident.
7. Completes required documentation in an accurate and timely manner.
8. RN oversees care delivery by Licensed Practical Nurses and Geriatric Nursing Assistants/Certified Medicine Technician.
9. LPN RN oversees care delivery by Geriatric Nursing Assistants/Certified Medicine Technician.
10. Attends and/or participates in rehab team conferences regarding resident progress, problems and needs.
11. Acts in compliance with Broadmead regulatory and professional standards and guidelines.
12. Assesses continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical currency.
13. Adheres to facility policies and procedures and participates in facility quality improvement and safety programs.
14. Abide by the Neighborhood Approach to caring for people living with a memory challenge or diagnosis, to include:
a. Being present during all meal times to assist with dining services, bring residents to eating areas or delivering to resident’s rooms.
b. Ensuring that special diets are served in accordance with the prescribed diet, including fluid, responsible for meal accuracy prior to delivery.
c. Monitor all likes and dislikes and problems with chewing/swallowing etc.
d. Communicate pertinent information daily to dietary Aide, supervisor and/or dietician.
e. Assist with serving meals and beverages collaboratively with the Host/Hostess or as needed based on resident request.
f. Ensure proper server and food handling.
15. Performs other duties as assigned.
16. Accountable for promoting internal customer service.
