**TITLE**: Liaison - Community (Toyota - Driving Possibilities Partnerships)

**JOB DESCRIPTION**:
Facilitate communication and bridge the gap between home, school, and community. Build a positive environment for communication between home and school. Maintain appropriate records of liaison activities.

**QUALIFICATIONS**:

- Education and/or experience equivalent to High School Diploma or GED
- One (1) year working in a culturally diverse setting in a liaison capacity
- State of Arizona Identity Verified (IVP) Fingerprint Clearance Card
- Valid Arizona Driver’s License

**ESSENTIAL FUNCTIONS**: (Essential functions, as defined under the Americans with Disabilities Act, may include any of the following tasks, knowledge, skills, and other characteristics. **DUTIES AND RESPONSIBILITIES**:

- Perform attendance verifications.
- Initiate and maintain contact with district and/or school personnel.
- Work with parents and students to improve attendance.
- Conduct home visits and confer with parents via telephone.
- May be required to control certain behavior(s) through physical restraint.
- May interpret materials and notices and translate between school personnel, students, and parents, if bilingual.
- Maintain records of liaison activities and home visits.
- Prepare various reports as required.
- Provide transportation to parents/guardians and students to school meetings, clothing bank, and medical/social services appointments as needed.
- Assist parents in completion of forms, questionnaires and other necessary documentation to enroll students.
- Assist school personnel to identify students in need of supportive or special services.
- Assist with the upkeep and disbursement of clothing/shoes/hygiene items through the CWK clothing bank to provide for students’ needs.
- Coordinate and maintain community outreach projects/resources by participating and acting as lead in assigned committees.
- Maintain daily calendar and Student Contact Log that accounts for time and services.
- Follow state law and district policy with cases involving child abuse and neglect.
- Maintain accurate records for compliance with district, federal guidelines.
- May be required to participate in student’s meetings which occur beyond the staff’s work assignment schedule.
- Perform other duties as assigned.

**KNOWLEDGE, SKILLS AND OTHER CHARACTERISTICS**:

- Knowledge of principles, practices and methods of acting as a liaison
- Knowledge of district policies and procedures
- Knowledge of community agencies available for referral
- Knowledge of special education laws, procedures, practices, materials, equipment and compliance standards required to provide appropriate learning experience for special education students
- Knowledge of the nature and effects of impairment(s) in all areas of disability
- Knowledge of the objectives and activities of a school system
- Knowledge of the keeping of necessary records and files
- Skill translating orally and in writing
- Skill demonstrating proficiency in district designated computer hardware and software including Microsoft Office, MET and IEP software programs and other relevant software
- Ability to obtain and maintain NCI, First Aid, and CPR certification
- Ability to drive from campus to campus and to student homes
- Ability to maintain a valid AZ Driver’s license
- Ability to establish and maintain effective working relationships with peers, parents and the community; excellent human relations skills; skill in both oral and written communication; ability to work in multicultural environment with diverse population of students, staff and parents
- Ability to coordinate activities with related and specialist staff
- Ability to develop and maintain accurate records and reports for compliance with district, federal, and state guidelines

**WORKING CONDITIONS**:
Position is exposed to potential infectious disease in transporting and investigating students.

Position may be exposed to potential physical harm from student and/or parents in dealing with attendance problems.

**PHYSICAL EFFORT**:
Position is generally sedentary with lifting limited to less than 10 lbs.

**WORK YEAR**: 218 Days

**DEPARTMENT/SCHOOL**: Student Success Enterprise

**GRANT FUNDED POSITION**:This is a grant funded position. Continuation is contingent on future grant funding.
