Medical Social Worker

The medical social worker is responsible for coordinating patient care through the Long-Term Quality Care Navigation Program by developing effective partnerships with patients, their caregivers/families, community resources and providers. Through these partnerships, the medical social worker promotes high-quality care that is patient and family centered within and across the long-term care setting.

Below are the roles and responsibilities that the medical social worker is expected to fulfill to help facilitate quality, patient-centered care:

- Facilitate patient access to appropriate medical and specialty providers as well as other team support.
- Develop care plans based on mutual goals with patient, family, physician, and documentation in medication record. Promote patient compliance with care plan and revise the care plan as needed.
- Collaborate with patients, their families/caregivers, healthcare professionals and community resources to develop a comprehensive plan of care that promotes health and meets the patient’s care goals.
- Provide timely communication and follow up with patients, families, and healthcare providers regarding changes in patient condition.
- Identify changes in the patient’s overall health that would warrant further assessment from a clinician.
- Conduct PHQ-9 and BIMS assessments in collaboration with the practitioner.
- Serve as a point of contact, advocate, and informational resource for patient, family and care team for coordination of healthcare resources as needed.
- Assess the patient’s and family’s unmet health and social needs.
- Assist patient and family with adherence to follow-up appointments.
- Contacts care providers and patients regarding day-to-day changes.
- Collaborate and communicate well with outside providers and healthcare staff regarding mutual patient care.
- Utilize tools and documents that support a chronic care management model.

Pay: $38.00 - $41.00 per hour
