**INTRODUCTION**:
Central Neighborhood Health Foundation is a Federally Qualified Healthcare Center committed to the Triple Aim as described by the Institute for Healthcare Improvement.

Improving the US Health Care System requires simultaneous pursuit of three aims: 1) improving the experience of care, 2) improving the health of populations and 3) reducing per capita costs of health care. Preconditions for this include the enrollment of an identified population, a commitment to universality for its members, and the existence of an organization (an "integrator") that accepts responsibility for all three aims for that population.

**SUMMARY**

The PrEP Navigator will report to the HIV Prevention Program Coordinator and will provide intensive care coordination, support, and services to HIV negative individuals who require assistance in accessing and remaining in PrEP care. The PrEP Navigator will work closely with the HIV Prevention Program Coordinator to perform PrEP and PEP education, engagement, patient navigation and adherence counseling. The Navigator will conduct client centered PrEP/PEP education and adherence counseling services with a focus on patient acquisition, engagement and retention. The PrEP Navigator will assist patients with such services as, but not limited to, health insurance enrollment, PrEP provider identification, appointment scheduling, appointment reminders, education on the benefits of PrEP and ongoing maintenance.

**ESSENTIAL DUTIES AND RESPONSIBILITIES **include but are not limited to the following functions.
- Coordinate with the HIV Patient Education Specialist to initiate access to PrEP care and provide assistance to existing PrEP patients.
- Work directly with patients seeking consistent access and coverage for PrEP medications.
- Ensure that PrEP patients receive relevant HIV/STI-related education and services, sexual health information, and navigation to health insurance as applicable.
- Assist in the implementation of PrEP services and appointment scheduling in primary care for PrEP.
- Provide individual and relationship based HIV and STI counseling.
- Provide education and Post-Exposure Prophylaxis (PEP), and other effective biomedical and prevention interventions.
- Refer eligible, interested individuals to primary care medical services at CNHF to further discuss PEP and other preventative care.
- Coordinate collaborative relationships related to PrEP implementation and service delivery, focusing on agencies located in and around clinics.
- Review and maintain current and accurate knowledge of emerging scientific research regarding biomedical HIV/STI prevention strategies, including PrEP, PEP and related interventions.
- Prepare weekly and monthly reports to facilitate accurate and timely reporting of PrEP navigation activities.
- Maintain accurate and confidential records of all testing information.

**EDUCATION and/or EXPERIENCE**
- Possess a Medical Assistant diploma.
- Minimum of 2 years working as a Medical Assistant.
- Experience with HIV/STIs services is preferred.

**SKILLS**
- Must have a high level of accuracy, time management skills, and possess strong organizational skills.
- Demonstrates great problem-solving, leadership, and organization skills.
- Able to work well under tight deadlines and respond to rapidly changing demands and provide efficient follow up.
- Able to communicate clearly and concisely, both verbally and in written correspondence.
- Ability to work independently and within a group setting.

**POLICY**
