**Job Title**: CBRF Caregiver

**Job Description - **Providing total care services to residents in our 8 bed CBRF Facility by assisting them with Activities of Daily Living such as self-care, medication administering, clothing/bathing, food/meal prep, leisure time/social companionship, travel to and from appointments as needed, completing accurate documentation for shift rotating communication, personal and social adjustment skills in a home environment that is set forth by Good Hope Manor standards.

**Requirements**:

- Completion of a state approved CBRF Courses, medication administration course, standard precautions course and currently on registry - Required.
- High School Diploma or GED.
- CNA license
- Preferred.
- Two years of caregiver experience in healthcare facility/advanced residential care experience in a CBRF or equivalent type of group home environment, and or training combined.
- Ability to work in unsupervised setting by leading with maturity and integrity.
- Flex shift rotation on weekends. - Required.

**ESSENTIAL DUTIES AND RESPONSIBILITIES**:

- Responsible for providing direct care as outlined in the care plan in accordance with company policy.
- Provide direct medical care of on an advanced level to include G-tube feeding, working with ileostomies, diabetic medication and injections, and other related medical procedures on a high level. We have multiple Hoyer-lift residents. May require advanced certification in insulin administration. Must be able to react and provide care in emergencies whether working with other staff or when working alone.
- Responsible for reading and remaining current with all resident files, posted employee notices and other communications that pass from shift-to-shift.
- Provide care for residents including bathing, dressing, grooming hair, oral care, skin care, peri-care and shaving in additional to required health care.
- Provide and assist with elimination needs through proper use of bed/urinal, incontinent pads/linens and assist with bowel and bladder programs, as required.
- Provide daily assistance with mobility. Assist with ambulation, rehabilitation needs, transfers and range of motion according to care plan. Use of proper body mechanics and Hoyer/Arjo lift. Includes proper use of supportive devices, gait belt, wheelchair, bed and other assisted moving devices to ensure the safety and comfort of residents.
- Provide for resident comfort needs including proper positioning in wheelchair and bed including turning, repositioning as needed, and massage with individual privacy.
- Complete daily tracking of resident goals through written documentation through medication logs, menus, daily progress notes, house communication log and other written documentation.
- Responsible for menu planning and meal preparation. Includes tube feeding and working with feeding equipment and pumps. Includes feeding of residents and provision of any needed assistance. Observes all safety requirements to reduce the potential for injury, choking and exposure to any food substances that produce allergic reactions.
- Responsible for completion of laundry and cleaning duties as scheduled for the shift worked. Requires the ability to change and improvise such duties as required depending upon the needs of the residents, which take priority.
- Conduct and participate in emergency response drills (i.e., fire, severe weather, etc.) as required and completion of required emergency response documentation.
- Provide mature role model for residents and never leaves residents unattended at any time.
- Responsible for completing and charting on tasks involving the Resident Activity Program.
- Must be able to complete activities such as sweeping/mopping, cleaning of floors and lower cabinets, dusting including ceiling corners, reaching into elevated spaces such as cupboards, closets, etc. for cleaning and placement/retrieving of items, manual dexterity to fold clothing, manipulate and open medications, write and place items with accuracy. As needed. Blood pressure measuring equipment, thermometers, weight scales and X other such equipment.
- Requires employee to be a team player and cooperative with other staff members. Requires the use of tact and maturity when dealing with a wide variety of interactive issues involving different personalities and behaviors.
