The _**Behavioral Health Case Manager**_ will educate and serve as a resource to providers and patients who need support for chronic disease management with a focus on mental health, substance abuse, social-emotional, and/or environmental issues. This position will provide support and intervention face-to-face, via telehealth and on the phone. This person will work in a manner that supports both the patient and the health center’s mission and performs in accordance with system-wide competencies and behaviors.

ESSENTIAL DUTIES AND RESPONSIBILITIES
- Communicates with primary care providers in order to educate them concerning behavioral health disorders and treatment option availability for the patients they serve.
- Serves as a resource between the primary care provider and the patients’ behavioral health provider to improve coordination of services and information flow.
- Provides self-management support.
- Provides ongoing phone contact with patients.
- Provides coordination and linkage to general medical services.
- Works with patients’ providers to customize services to best meet patients’ needs within the parameters of available services.
- Coordinates, reviews and maintains daily logs for reporting purposes and for weekly review.
- Meets with patients, primary care and behavioral health providers, and other staff as needed to develop and coordinate treatment plans; meets with patient family members if needed.
- Interfaces with and refer patients to other support services as appropriate.
- Attends regular meetings with the provider team.
- Participates in QA/QI activities and provider training, along with professional development activities.
- Performs other duties as assigned.

KEY COMPETENCIES & ABILITIES
- Relevant experience in case management.
- Ability to interact with physicians & other health care professionals in a professional manner.
- Working knowledge of physical health and behavioral health medications.
- Proficient in Microsoft Office
- Keeps all matters related to the organization confidential in accordance with HIPAA and the confidentiality policies and procedures.
- Training and/or experience with motivational interviewing, behavioral activation or health coaching.
- Previous experience in crisis intervention is preferred.

EDUCATION & TRAINING REQUIREMENTS
- Masters Degree in Social Work, or BS level with relevant psychosocial experience and/or a current RN license.
- Current CPR/Basic Life Support Certification upon hire or willing to obtain & renew as needed.
- A minimum of 5 years’ experience providing case management services.
- A minimum of 3-5 years’ experience working with substance abuse services.
- Position must keep current on information pertaining to patients, family & community need.

Pay: $54,200.00 - $67,200.00 per year
