**Hospital Liaison Qualifications & Responsibilities**:

- Foster collaborative relationships with hospital discharge planners and medical professionals.
- Conduct assessments and evaluations of potential residents in hospital settings.
- Coordinate smooth transitions for patients from hospitals to our nursing home.
- Educate healthcare providers and community partners about our facility's services and capabilities.
- Act as a liaison between the nursing home and external healthcare entities.
- Bachelor's degree in healthcare administration, business, or a related field (preferred).
- 3-5 years Previous experience as an Admissions Director or Hospital Liaison in a skilled nursing/LTACH, or acute rehab hospital desired.
- Strong communication and interpersonal skills.
- Knowledge of healthcare regulations and standards.
