Job Summary - Intake Coordinator Sun-Thur 7am-3pm

The Intake Coordinator is a Recovery Specialist with additional assigned duties. The Intake Coordinator is responsible for working with the Program and Clinical Director to triage, interview, and intake new residents as well as to maintain the program and BSAS waitlist management system.

**Responsibilities**:
1. Obtain and complete all pre-entry documents.

2. Review referrals for appropriate documentation.

3. Conduct assessments with appropriate applicants and review them with the Clinical Director.

4. Maintain a list of all applicants that includes scheduled assessments, waitlist for accepted applicants, denials and applicants removed from the waitlist.

5. Maintain BSAS and program waitlist.

6. Assist referral sources with coordinating medication refills and methadone dosing prior to a new residents’ intake to the program.

7. Utilize LHI’s Electronic Health Record to complete required documentation prior to admitting new residents.

8. Explain program rules and polices to all applicants during their assessments.

9. Assist residents in completing necessary paperwork, including signing releases of information and other consents for treatment

10. Responsible for accurate and timely clinical documentation for services

11. Conduct substance use and mental health assessments, utilizing standardized assessment tools and motivational interviewing techniques.

12. Pursue ongoing professional development relevant to the job position.

13. Participate in clinical supervision regularly as well as interdisciplinary team meetings.

14. Effective and timely communication and networking with external referral sources.

15. Supervise and monitor the health and safety of residents in the program.

16. Completion of daily monitoring and communication log.

17. Assisting residents in following daily schedule and routines.

18. Responsible for maintaining the staffing ratio as assigned by the Program Director; (remaining on-site as directed by supervisor).

19. Attending and participating in weekly staff meetings, as assigned by the Program Director.

20. Actively promote recovery.

21. Cooperate with clinical staff to ensure effective and efficient programming.

22. Answering telephone.

23. Assist with resident needs and daily requirements.

24. Assist with daily operations such as food shopping, meal preparation, monitoring of chores, household tasks as assigned.

25. Participate in individual and group supervision as assigned by the Program Director.

26. Participate in your performance evaluation with the Program Director.

27. Have knowledge and understanding of DPH regulations.

28. Adhere to all Programmatic Policies and Procedures, including Driving Policy.

29. Occasional transportation of residents is possible.

30. Adhere to LHI employee handbook rules.

31. Appropriate utilization of on-call procedures.

32. Responsible for the immediate notification of safety/health/legal issues happening in the program (resident or programmatic), including completing an Internal or Critical Incident Report form.

33. Complete agency training to utilize electronic health records and complete appropriate services as assigned.

34. Responsible for timely calling out to identified supervisor due to inability to cover the shift.

35. Responsible for completing and authorizing weekly timesheets using our online portal.

36. Provide Crisis Intervention (resident and programmatic issues)

37. Utilizing overdose and prevention training (Narcan) if needed.

38. Responsible for completing both in person and online trainings as assigned monthly.

39. Responsible for the cleanliness of office and communal spaces.

40. Adhere to all HIPAA and 42.CFR guidelines.

41. Complete medication training with program nurse or Medication Specialist.

42. Adhere to all programmatic medication policies and procedures.

43. Observe and document residents' self-administration of medications.

44. Complete daily medication counts.

45. Other duties as assigned.

Required education and experience

1. High school diploma or GED certificate

2. 1+ years working in addictions/mental health field
