**Description**
The Medical Social Worker reports to the Director of Case Management or designee. Responsible for providing social services to patients and their families. Provides clinical social work services in a manner consistent with the philosophy and objectives of Providence Health & Services and the NASW. The Social Worker duties include but are not limited to providing resources, completing bio-psychosocial assessments, participation in treatment team planning, and discharge planning/case management service & crisis intervention. Social Worker evaluates patients on behalf of the ministry within emergency department if patients meet criteria for involuntary Hospitalization Holds (5150) and voluntary placement.

**Required qualifications**:
- Master's Degree in Social Work or related field.
- California Fire and Life Safety Card - National Organization within 30 days of hire.
- 1 year or more of medical social work within a hospital (psychiatric or medical).
- Computer experience. Computer literacy.
