As an RN in our home health division, you will play a crucial role in delivering skilled nursing care and support to patients and their families.
**Responsibilities**:
- Perform comprehensive nursing assessments and develop individualized care plans in collaboration with the interdisciplinary team.
- Provide skilled nursing care, including wound care, medication administration, and patient education.
- Monitor patients' vital signs, symptoms, and overall health status, and communicate any changes to the healthcare provider.
- Assist with activities of daily living, such as bathing, grooming, and mobility, as needed.
- Document all nursing assessments, interventions, and patient responses accurately and in a timely manner in accordance with regulatory standards and agency policies.
- Educate patients and their families on disease management, medication administration, and self-care techniques.
**Requirements**:
- Current unrestricted Registered Nurse (RN) license in the state of California.
- Minimum of one year of experience as an RN, preferably in home health or a related field.
- BLS certification required.
- Strong clinical assessment and critical thinking skills with the ability to make sound nursing judgments.
- Excellent communication and interpersonal skills.
- Reliable transportation and a valid driver's license.
- Ability to work independently and as part of a multidisciplinary team.
**Job Types**: Part-time, PRN, Per diem
Pay: From $80.00 per hour
