**Responsibilities**: As a Home Health Registered Nurse, you will:

- Conduct admission, case management, and follow-up skilled nursing visits for home health patients.
- Administer ongoing care and case management, following directions from the Clinical Manager/CCO.
- Coordinate appropriate care, collaborating with various healthcare professionals (e.g., Physical Therapists, Occupational Therapists, Home Health Aides, and external providers).
- Provide regular updates on patient care, condition, and progress to the patient's physician and Clinical Manager/CCO.
- Work with patients and families to implement care plans, aiming for optimal resolution of needs and functional improvement.
- Supervise Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing clear written care instructions reflecting the current plan of care.
- Facilitate patient discharges after consulting with the physician and Clinical Manager/CCO, ensuring all necessary clinical documentation is prepared and completed.

**Qualifications**:

- Diploma, Associate, or Bachelor Degree in Nursing required
- Minimum of one year nursing experience preferred
- Strong med surg, ICU, ER, acute experience a plus
- Home Health experience a plus
- Current & unrestricted Registered Nurse/LPN licensure
- Current CPR certification
- Strong organizational and communication skills
- Valid driver’s license and car insurance required

**Compensation**:$42.00+ per visit for RNs/ $37.00 per visit for LPNs **(average 35-40 visits per week for full-time nurses)
