**Title**:

- Home Health Registered Nurse

**Responsibilities**:

- Performs initial and follow-up assessments including vital signs and medication review
- Initiates the plan of care and makes necessary revisions as patient status and needs change
- Assesses patients’ condition during every visit and put your observations into chart
- Provides skilled nursing care in accordance with physicians' order and in compliance with the state and federal regulations
- Initiates referrals to ancillary disciplines (PT/OT/MSW) within 48 hours if needs
- IV infusion, Injections, Medication administration (Administer medication as prescribed by the patient’s Physician)
- Wound Care / Wound Vac (Dress or redress open wounds and assess progress of healing)
- Listens to the patient and responds to concerns or requests (Educate patients and their families on proper home health care strategies and procedures)
- Provides health care instructions to the patient as appropriate per assessment and plan of care
- Coordinates with Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient’s care plan
- Makes recommendations for devices or tools that might improve the patient’s quality of life
- Acts as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility to coordinate patient care for assigned caseload
- Ostomy, Colostomy, Ileostomy Care
- Diabetic Care / Teaching
- Blood Draw / Lab work
- G-tube care and management
- Cardiac Care, CHF management
- Pre and Post - Surgical Care
- Pain management

**Job Types**:

**Requirements**:

- At least one year experience working at a home health agency
- Current RN license in California
- Current CPR certification
- Current Physical Exam
- Current PPD or X-Ray Exam
- Liability insurance
- Valid driver's license
- Auto Insurance
