**EDUCATIONAL QUALIFICATIONS AND REGULATORY REQUIRED**:

- Demonstrated ability to read, write and communicate effectively.
- CNA license or 6 months of continual care in delivering personal care tasks in family care, primary home care, or personal assistance services.
- IF a CNA, Must not be designated by an HHSC caseworker on HHSC's Authorization for Community Care Services form as "Do not hire."

**PERSONAL ASSISTANT ESSENTIAL TASKS**:

- Provide administrative support to the individual, including managing calendars, scheduling appointments, organizing meetings, and taking notes during meetings.
- Support communication and management of household vendors, including interfacing with house keeper, maintenance professionals, and assist follow up appointments for the agency.
- Support physical and healthcare regimen including attendance at medical appointments, workouts, dietary health, medication management, and support with occupational therapy exercises.
- Transporting the client in their vehicle to daily activities and appointments.
- Booking and arranging travel, transport and accommodation.
- Maintain a schedule and calendar for client’s personal and professional use.
- Communicate and relate well with the client and family.
- Discretion and confidentiality of the individual’s and family’s privacy is mandatory.
- Assist individual with time management skills

**CLIENT CARE SPECIALIST ESSENTIAL TASKS**:

- Assists with or/or provides personal care duties as outlined in the Plan of Care or Individual Service Plan and assigned by the Supervisor, i.e. Assisting with basic hygiene and grooming duties to promote adequate self-care.
- Assisting client with self-administered medications:

- MAY NOT FILL CLIENT’S MEDICATION BOX
- Assisting with medication includes reminding; getting a glass of water; bringing medication containers to a client and or opening medication-planners for the client.
- Household management tasks such as loading/unloading the dishwasher, running errands including grocery shopping, managing online orders, family laundry, and other organizational projects.
- Routine household chores as requested by the client or family.
- Shopping for clients. The company does not reimburse the provider for gasoline
- Shopping includes preparing a shopping list; going to the store and purchasing or picking up items; putting food away; picking up medication
- Reports any abnormal conditions to the appropriate supervisor, including any unusual behavioral or medical changes in the client.
- Reports specific situations to the appropriate supervisor concerning the client’s care:

- changes in the individual's needs;
- incidents that affect the individual's condition;
- hospitalization of the individual;
- the individual's absence or relocation from home;
- the attendant's inability to work; and
- suspicions or allegations of abuse, neglect, or exploitation of the individual.
- Ensures that the rights of the client are maintained.
- Reports to the appropriate supervisor in advance of changes to the schedule or absence from work.
- Attends staff conferences and in-service meetings as scheduled.
- Represents the Agency in a loyal manner and dresses in accordance with the dress code of the company.
- Completes service delivery record (timesheet) on the day of each visit and turns in bi-weekly timesheet timely after the last day worked during the pay period
- Uses the EVV/Alternative Device consistently and accurately to record time worked for each client’s scheduled hours if required to do so by the payer.
- Perform other health-related tasks which may be delegated by a physician, an R.N., or other healthcare professionals.
- Other duties as assigned.

**PHYSICAL QUALIFICATIONS**:

- Requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye-hand coordination.
- Requires prolonged standing or sitting
- Requires frequent bending, stooping or stretching.
- Requires corrected vision and hearing within functional levels and the absence of color blindness.
- Able to verbally communicate effectively.
- Requires daily documentation
- Able to work in stressful environments.
- Able to work the hours required for duties and responsibilities.
- Must be able to drive
