**Responsibilities**

Provide holistic nursing care to patients in the hospice program, addressing physical, emotional, and spiritual needs.

Conduct thorough assessments of patients and develop individualized care plans in collaboration with the interdisciplinary team.

Administer medications and treatments as prescribed, and monitor and manage symptoms to enhance the quality of life for patients.

Educate patients and their families on the hospice philosophy, treatment options, and end-of-life care decisions.

Collaborate with physicians, social workers, chaplains, and other team members to coordinate and implement the patient's plan of care.

Offer emotional support and counseling to patients and their families, promoting dignity and respect during the end-of-life journey.

Document patient assessments, care plans, and interventions accurately and in a timely manner.

Participate in regular team meetings, case conferences, and educational sessions to enhance knowledge and skills in hospice care.

Ensure compliance with all regulatory requirements and standards of practice related to hospice care.

Maintain open communication with patients, families, and the interdisciplinary team to promote effective care coordination.

**Qualifications**:
Current Licensed Vocational Nurse(LVN) license in Texas.

Minimum of 2 years of nursing experience, with at least 1 year in hospice or palliative care.

Excellent communication and interpersonal skills.

Compassionate and empathetic approach to patient care.

Ability to work collaboratively in a multidisciplinary team.

Knowledge of end-of-life care principles and pain management.

Strong organizational and time-management skills.

Willingness to participate in ongoing education and training.

Pay: $28.00 - $32.00 per hour

Expected hours: No more than 40 per week
