**Primary Job Duties and Responsibilities but are not limited to**:

- Provide one-to-one support as needed in a variety of settings (home, community).
- Provide physical and cognitive support to individuals with developmental disabilities to help them lead better lives
- Be a participating member of the individual’s team (circle of support).
- Ensure the health and safety of the individual by maintaining proper supervision and support at home, during transport, and out in the community.
- Demonstrate a willingness to creatively work on the valued outcomes that are important to the individual which may include self-care, independent living skills, recreation, job readiness, and community integration.
- Transport individuals to activities, classes, and outings in their community.
- Provide a positive model for the individual when out in the community.
- Building and maintaining positive relationships with the individuals
- Engage in mutually respectful partnership/relationships with the individuals, family members, co-workers, and others
- Support the safety of all individuals in everyday situations
- Assist in the development of skills to help the individual meet the productivity expectations of the workplace
- Act as a role model for positive behavior.
- Effectively communicating supportive feedback to the individual when they are experiencing emotional distress.
- Ensure there is consistency to the individual’s daily routine.
- Complete all OPWDD mandatory training.
- Complete accurate and timely documentation of services provided.
- High school diploma or GED preferred
- Experience working with individuals who have developmental disabilities.
- Direct Care experience preferred.
- Must be able to read, write, and comprehend English effectively
- Must be able to utilize electronic health systems
- Must have a cell phone for electronic health systems
- Must maintain a valid NY State driver’s license and maintain a clean driving record. ( If Required)
- Must have a philosophy that is committed to the Mission, Vision, and Core Values of Unique Life
- Ability to maintain confidentiality
- Must be a positive team player and maintain a positive attitude
- Skills in planning, organizing, problem-solving, and conflict resolution.
