**NAVIGATOR (PSN)**

PROGRAM: HIV Prevention

**CLASSIFICATIONS**:Full-Time, 35 HOURS/WEEK

Founded in 1981, **Diaspora Community Services (DCS)** is a Brooklyn-based multi-service and multi-cultural 501 (c) 3 nonprofit providing comprehensive supports to low income residents, immigrants, and the chronically ill. DCS serves some 10,000 annually through three programmatic sites in New York City. Staffed by seasoned professionals in the health and social service sectors and supported by a dedicated Board of Directors as diverse as the vibrant communities we serve, DCS provides robust programming that targets the most vulnerable of our communities.

DCS’s programs address the very root causes of the challenges our clients face, empowering low income families and individuals to overcome obstacles to their well-being and self-sufficiency.

**Job Summary**:
**Reports to**:Senior Manager of Programs

**Responsibilities include but are not limited to**:

- Develop and conduct recruitment strategies to persons who identify as LGBTQIA, homeless, and/or new immigrants in designated catchment areas in Brooklyn
- Coordinate linkage to public benefits
- Provide outreach services in the community to inform others about HIV/HCV prevention services and serve as link between clients and appropriate services
- Conduct testing, pre and post testing counseling, and risk assessments to priority populations
- Manage and develop culturally and linguistically appropriate social marketing messaging and materials to raise awareness around substance abuse, HIV, and HCV
- Provide evidence based-based programming to clients who identify as LGBTQIA to ensure effective implementation of intervention strategies to high-risk population
- Coordinate education and technical assistance opportunities to individuals in various community based settings to promote importance of screening for HIV/HCV and assuring clinical care needs of clients are met
- Work alongside program team to develop and streamline navigation referral process for individuals in need of SUD or HIV/HCV services
- Conduct ongoing support to clients such as risk behavior screenings, scheduling assistance, and coordination with providers to encourage retention in services
- Assist the Program Manager on projects as needed and attend relevant State and Coalition trainings
- Analyze pre and post data from EBPs and from referrals for substance abuse, mental health services and primary care services to develop strategies to improve program effectiveness
- Assist with data entry and maintain client files, statistical data, and referral logs
- Travel throughout all 5 NYC boroughs as needed
- Other program duties as assigned

**Qualifications**:

- Associate, Bachelor’s Degree in Public Health, Social Work, or related social science field preferred
- Capacity to conduct street and community outreach targeting populations
- Knowledge and skills in working in a team effort and demonstration of a strong commitment to team work among staff
- Knowledge of public benefits and experience in connecting individuals to ensure access
- Experience in HIV/HCV testing (pre and post counseling preferred); with knowledge of Ending the Epidemic (ETE) principles and U=U (Undetectable = Untransmittable)
- Possess good communication and oral presentation skills
- Must be able to work flexible hours; this includes are normal business hours and weekends

**Compensation**:
Compensation is competitive and commensurate with experience. Diaspora Community Services offers a comprehensive benefits package including full health, dental and vision insurance benefits, voluntary flexible spending plan, 403b retirement plan, and eligible to accrue 10 vacation days, 3 personal days, 8 sick days and up to 12 paid holidays.
