Position Summary

The Outreach Specialist is responsible for serving as the initial and main point of Field contact between the Client and current Enrollees and ensures appropriate utilization of services that assists in achieving desired health outcomes.

Scope: Community-based Face-to-Face Outreach in Washington DC

**Responsibilities**:

- Complete Initial screenings for any Enrollees who are within the ninety (90) day window of enrollment.
- Problem solve and address barriers, recommending and engaging resources as appropriate to address identified risk factors through best practice and evidence-based interventions.
- Record details of attempts, interactions, services rendered to Enrollee, and future recommendations.
- Support Enrollee access to care through home visits, processing of reports, and distribution of collateral materials.
- Perform new member orientations.
- Provide superior, professional, and courteous service to customers.
- Participate in required trainings and monthly meetings.
- Meet and exceed established performance metric standards.

Knowledge, Skills and Abilities
- Ability to understand the basic principles of managed care and the concepts of publicly financed health insurance such as Medicaid and SCHIP programs is preferred.

Qualifications:

- High School Diploma or GED required.
- Community Health Worker designation preferred.
- 1 + years of experience in customer service or any combination of education and experience that would provide an equivalent background.
- Must have self-motivation and initiative with the ability to work independently.
- Incoming new hires will need to provide verification of their vaccination status within 60 days upon hire if start date is between September 1 and March 31. Those hired from April 1 - August 31 will not be required to verify vaccination status until the following annual vaccination campaign.
- Must possess and maintain a valid driver’s license with a clean driving record, as well as have personal vehicle for transportation to/from the job site.
