**Requirements**:
- Valid RN license and BLS certification
- Proven experience in case management or related nursing roles
- Strong knowledge of medical records management and patient care protocols
- Ability to work effectively in a fast-paced healthcare environment
- Excellent communication and interpersonal skills for patient interactions
- Familiarity with utilizing electronic health record systems for documentation
