**Minimum Qualifications**:

- Master of Social Work preferred or Bachelor of Social Work or a Bachelor’s degree plus at least one year of supervised experience in human services.
- Bilingual Preferred: English/Chinese, English/Spanish, English/Polish, English/Russian.
- Temporary position (February to May 2024) Monday to Friday 9:00am to 5:00pm
- Conducts in-home assessments and reassessments, develops and implements care plans, provides information, referrals, assistance with obtaining benefits and entitlements, advocacy and follow-up on care plans.
- Monitors the delivery of services (i.e. meals on wheels and home care) and advocates on behalf of clients.
- Inputs all required data into the STARS system.
- Provides crisis intervention, including client no-entry follow-up, and provides on call assistance after 5:00 p.m. and on weekends in the event of a city-wide emergency (i.e. heat advisory, hurricane, flooding, etc.)
- Performs tasks as required by the Case Management Program Supervisor, Program Director & Director of Programs to support the functioning of the program.
