Job Details:
The **Home Health Aide Coordinator** responds appropriately to the special needs of every patient while maintaining confidentiality and compliance with the HIPAA Compliance Program. Responsibilities include, but are not limited to:

- Coordinating the placement of home health aides as indicated by the hospice nurse
- Working with home health aide vendors to place aides as needed
- Maintaining updated vendor aide files; NYS HHA Certificates, Chauncey Reports, in-services, etc.
- Interacting with patients and family members regarding aide placements, scheduling, and addressing any problems or issues

**Minimum Requirements**:

- High school diploma or equivalent required
- Commitment and empathy to patients and family
- Good communication and pc skills

Posted Salary Range: USD $25.00 - USD $29.00 /Hr.:
