Case Manager Mental Health Professional

We are looking for a **Case Manager - Mental Health Professional
- HOPWA in Northern Kane/Elgin, IL** Are you the right fit?
**What will you be doing?**
- The HOPWA Case Manager is responsible for managing individuals’ housing including: all paperwork needed to remain in compliance with TCHIP/HOPWA program requirements/rules, assisting individuals in finding and obtaining appropriate housing, maintaining leases, developing and maintaining good working relationships with landlords, providing medical case management and behavioral health services as appropriate.
- The Case Manager is expected to obtain and maintain all needed trainings to remain in compliance with TCHIP/HOPWA requirements. The Case Manager is expected to participate in ongoing meetings and any other required committees needed to facilitate good working relationships with the HOPWA team as a whole including surrounding area HOPWA teams.
- The Case Manager must complete documentation of services including mental health services and HOPWA related services provided in accordance with state and agency standards as applicable.
- The Case Manager is expected to work with a team of behavioral health service providers within the agency as well as outside agencies, if applicable, to ensure positive client-centered approach.

- Bachelor’s degree in human services or a related field (required)
- Experience working in social services (_preferred_)
- A valid Illinois driver’s license and automobile insurance
