Bilingual Field Supervisor

We currently have an opportunity for a **Field Supervisor**.
The Field Supervisor has primary responsibility for the in-home supervision of Homemakers and Home Health Aides as well as the on-going assessment and management of client situations/problems related to services provided. Involvement in teaching/training programs and documentation of activities is also a priority.
**Qualifications**
- Minimum of High School Diploma or equivalent degree
- At least 1 year experience in health care
- Excellent communication and interpersonal skills
- Strong conflict resolution skills
- Must be able to work in a variety of environmental conditions
- Ability to handle crisis situations as well as perform problem solving interventions
- A valid Driver’s License is required for this position as driving is a requirement
**Essential Functions**
- Complete Case Management documentation on all clients
- Schedule initial visits with clients to complete required admission and enrollment paperwork.
- Supervise field performance of Caregivers in-home to establish compliance with established plan of care and quality of services provided
- Demonstrate, as necessary, procedures of care to caregivers that are to be used for a client
- Complete, maintain, and submit reports and records documentation to the Home Care Manager on a consistent and as needed basis
- Provide guidance to Caregivers on providing patient care in adherence to the professional standards of the agency
- Assist Care coordinators in establishing immediate and long-term goals of the clients and in developing a Care Plan
- Establish and maintain satisfactory working relationships with clients, Caregivers and office personnel
- Performs other duties and activities as delegated by the Home Care Manager

Pay: $16.00 - $18.00 per hour
Expected hours: 40 per week
