Housing Case Manager

The housing case manager uses the housing first model and trauma-informed care approach to help adults experiencing homelessness find stable long term housing and connects program participants with other resources and programs that help them sustain long term housing and self-sufficiency. Case Managers assist clients staying at our temporary shelter/housing sites through advocacy, education, and support that will help them transition from homelessness to rapid rehousing and permanent housing programs. Case Managers may manage a caseload of 18-25 clients, as determined by the program's needs.

**Specific Duties, Functions, and Responsibilities**:

- Works with program participants to complete assessments, set goals, identify strengths and resources, and develop and implement weekly and long-term self-sufficiency service plans that are appropriate to their particular status and needs. Needs such as obtaining required and necessary identification and documentation for housing, education, work, treatment, and evaluating barriers to accessing services.
- Ensures interactions with program participants are client-centered, solution-focused and strengths based in all settings, language, and documentation.
- Under guidance from department/program leadership, coordinates appropriate referrals and assists program participants in accessing community supportive services and training that helps clients meet their goals. Coordinate with medical professionals and connect clients with crisis support services as needed.
- Coordinates with Front Steps colleagues, program managers; and other community partners to provide wrap around services to our program participants.
- Effectively use systems and tools, to ensure compliance with data standards and record-keeping as required by Front Steps policies, regulatory agencies, and grant requirements. Documents all client interactions, maintaining accurate notes and reports; and evaluates performance outcomes within required time frames.
- Participates in team and organizational meetings and assists in maintaining all procedural and documentation standards.
- Monitors expenses and works with program coordinators and managers to maintain service contracts/grants and audits.
- Provide transportation for clients to get to appointments and schedule in home visits as needed.
- Supports Front Steps in achieving their mission by helping with other projects, and special events.

- Strong teaching and learning principles to help clients build skills necessary to be more self-sufficient and take greater charge of their lives.
- Strong critical thinking skills that involve reasoning, organizing and analyzing information so problems are accurately understood and solutions are outcome-oriented and purposeful.
- Insightful of cultural diversity and work with a diverse population without judgement. Reflective and self-corrective by carefully considering meaning of data and interpersonal interactions; shows tolerance of different viewpoints; watchful of your own potential errors and bias that could be influencing thinking.
- Strong advocacy skills that help in connecting clients to resources while problem solving and negotiating between program participants and other service providers.
- Awareness of relevant laws, rules, and regulations that may influence eligibility or entitlement to services

**Minimum Qualifications**:

- 1 year experience with social services work or related field; or bachelor's degree in social work or related field.
- 1 year of professional experience providing case management services to individuals experiencing homelessness or closely related special needs population, and knowledge of community resources preferred.
- Reliable transportation and valid driver’s license.
- Knowledge of housing first, trauma informed care, and social service professional boundaries and ethics.
- Strong computer skills, including Microsoft, spreadsheets, database usage, and internet; or experience using a Homeless Management Information System (HMIS), ServicePoint preferred.
- Bilingual preferred (Spanish and English and/or ASL and English highly preferred).

**Preferred knowledge, skills, and abilities**:

- Case management experience working with the transitionally and chronically homeless or other special needs populations. Experience working with Veterans and their families highly preferred
