Children of the Rainbow is seeking an experienced and dynamic Case Manager to be part of a dedicated team of support staff committed to addressing the unique needs and experiences of young children who have faced trauma and adversity, with a focus on serving diverse communities.

Position Overview:
The Case Manager will provide culturally sensitive services to underserved individuals, including low-income and racially diverse communities.

Collaboration and Referral:

- Collaborates with Parent Coaches, mental health professionals, social workers, and counselors, to ensure comprehensive care for clients.
- Make appropriate referrals to medical providers, support groups, and community resources as needed.
- Communicate effectively with interdisciplinary teams to coordinate care and advocate for clients' well-being.
- During the first 6 months of employment the Case Manager will be responsible for a limited caseload as needed to receive certification and implement ABC with fidelity, equity, and through partnership with children and families most negatively affected by systemic oppression.

**Documentation and Compliance**:

- Maintain accurate and up-to-date documentation.
- Adhere to state licensing regulations, professional codes of ethics, and confidentiality guidelines, such as HIPAA (Health Insurance Portability and Accountability Act).
- Adhere to reporting and evaluation requirements as defined by Department of Health Care Services (DHCS), including informed consent, data collection and submission, and participation in evaluation.
- Collect and up-date clients charts.
- Preparing and processing documents.
- Gathering and organizing clients information.
- Seek referrals and resources.

The Case Manager promotes social and emotional well-being, of parents in their 3 trimester of pregnancy or who have children ages 0-2 years of age. In support of parents Coaches will implement **Attachment and Biobehavioral Catch-up (ABC)** an evidence-based home-visiting parenting program that partners with parents to support young children exposed to early adversity. As part of employment the Case Manager will receive training and certification through The University of Delaware Attachment Biobehavioral Catch-Up program. ABC Parenting trains and supervises home visitors in local communities in implementing ABC with fidelity, equity, and through partnership with children and families most negatively affected by systemic oppression.

Qualifications:

- High school diploma or equivalent required and some college coursework, associates degree preferred.
- Experience with case management and home visiting programs preferred.
- Strong understanding of cultural competency and ability to work effectively with diverse populations.
- Excellent communication and interpersonal skills.
- Compassionate, non-judgmental approach to services
- Bilingual and bicultural in English and Spanish.
- Verifiable experience supporting youth and families who are at high-risk for behavioral health conditions in socioeconomically disadvantaged communities.
