**About the Role**

The Social Adjustment Services (SAS) Coordinator will be the specialized member within Refugee Services that focuses on health and public benefits assistance for refugees. The position will work to promote self-sufficiency and goal attainment for clients, through connections to ongoing programming and mainstream supports, in compliance with Refugee Services programs and contract guidelines. This role will additionally help manage current and develop new resources within the community to support refugees, as well as support and assist with ongoing training and reviews that help maintain Social Adjustment Services (SAS) in compliance.

**Main Responsibilities**
- Provide direct service and health care coordination to newly arrived refugees, in homes, clinics, social service offices, and community health settings.
- Assist with outreach strategies to enroll clients into SAS program.
- Directly work with SAS staff to achieve expected outcomes and ensure high quality of services.
- Ensure compliance of SAS program with contract guidelines.
- Develop and maintain processes for quality control to ensure the case files of each client are up to date and ensure program compliance.
- Conduct intake and assessments to determine client eligibility for the program(s).
- Develop care plans with program participants, empowering them to reach client-centered goals and meet program guidelines.
- Encourage early self-sufficiency and independence of all program participants.
- Work closely with Refugee Services Programs colleagues to ensure that refugees are integrated into the broader array of services available in the organization.
- Promote SAS and Refugee Services Programs, provide advocacy regarding refugee health issues, and cultivate relationships for potential referrals with community partners, refugee community members, and other eligible individuals.
- Document all contacts and services in case files and complete assessments and outcome tracking within the computer and online databases.
- Assist in emergencies as they arise.
- Transport clients to and from appointments as necessary.
- Coordinate services with various community public health partners at the state, county, and local levels, as well as across programs.
- Serve as an advocate to increase awareness of refugee health issues within Refugees and the community.
- Maintain awareness of evidence-based best practices within the field of refugee health and provide staff with relevant information as it is learned.
- Work closely with SAS Program Manager to ensure compliance for SAS is met.
- Perform other duties as assigned.

**Qualifications, Skills & Experience**
- Bachelor’s degree is required, Master’s preferred.
- Two Years Experience working in case management or with refugee/immigrant populations is preferred.
- Excellent written and verbal communication skills.
- Must enjoy working in a diverse office environment with clients of all backgrounds.
- Energetic, self-motivated team player.
- Intermediate to advanced Microsoft Office computer skills (Outlook, Word, Excel, and PowerPoint).
- High degree of independent judgment, flexibility, and problem-solving skills.
- Must be able to work a flexible schedule including some late nights and weekends.
- Must be willing and able to drive 12 passenger van.
- Must have an automobile, valid driver’s license, and automobile liability insurance.
- Fluency in one of the refugee languages (Arabic, Spanish, Nepali, Kiswahili, and Burmese) preferred.
