**What You Will Do**
The Diabetes Outpatient Educator, under the direction of the Sharp Rees-Stealy Patient Education and Support Manager assesses the health and educational needs of adolescents, adults and geriatric patients, using specialized knowledge and skills, implements and evaluates that education on an ongoing basis. Provides support to related health care personnel and other entities regarding program development and departmental operations.

- Other : Education as required for Registered Dietitians (RD) under the guidelines of the Commission on Dietetic Registration (CDR).
- Certified Diabetes Care and Education Specialist (CDCES) - Certification Board for Diabetes Care and Education -REQUIRED
- CDR Registered Dietitian (RD) - Commission on Dietetic Registration (CDR) -REQUIRED

**Preferred Qualifications**
- Recent, related experience.

**Other Qualification Requirements**
- Current RD registration in good standing with the Commission on Dietetic Registration (CDR).

**Essential Functions**
- Educator

Ensures instruction to the patient and family based on age specific learning needs identified ie:
Geriatric: Evaluate cognitive ability, vision, hearing and dexterity as it relates to self blood glucose monitoring and medication administration. Wherever possible simplify regimes and give clear instructions on when to call for assistance.
Adult: Ensure instruction pertains to what learner needs to know ie; problem orientated and relates to learners own experience.
Keep lecture short and reinforce with printed materials or use of food models or audio/visual.
Ensure a simple diabetes regimen with emphasis that monitoring benefits the patient.
Give clear easily understood instructions.
Common problems/issues to observe for ie; Alcohol and drugs, eating disorders, puberty and poor control.
Assists with the development of activities and methods to ensure information is articulated and disseminated to staff.
Abides by the American Diabetes Association DSMES Standards as evidence by passing annual recognition requirements
Skills Roster: Performs skills according to Department Based Skills Roster.
- Financial responsibilities
Provides education, guidance, and recommendations to the payor regarding alternatives for care and services when appropriate.
Assigned cost center productivity measures are between 95%-100%.
Obtains appropriate registration and billing information ie; COA, copy of insurance card (both sides), Classification/Manifestation form, patient information form, and submits the information to the Financial Counselor within 48 hours, except when interrupted by a weekend but not exceeding 72 hours.
Variable expense per unit of service does not exceed budgeted expense unit of service.
Demonstrates/contributes cost-reducing or revenue-enhancing idea/actions.
In conjunction with Program Manager, develops fiscal year capital requirements plan.

Communicates accurately, effectively, clearly and concisely with others, both verbally and in writing.
Perceived by other healthcare workers as approachable when assisting in the achievement of established goals and objectives for improving clinical outcomes.
Develops positive collaborative relationship with other departments/entities/external healthcare agencies to facilitate and support quality care.
Identifies and directly addresses issues that affect patient care outcomes; collaborates with appropriate providers to prevent recurrence of issues and reports identified related problems using the supervisory communications structure.
Acts as a resource on complex patient care activities, which result in satisfaction among patient/family, and/or significant other.
Regularly attends and participates in team meetings which actively promotes the team concept to maintain quality of services and promote program growth.
Builds relationships and rounds consistently with management, clinical staff, physicians and other departments to facilitate the education process and evaluate educational outcomes.
Uses interpersonal communication strategies with individuals, as well as groups of patients/families/significant others, physicians and staff that result in:
