**RN Case Manager**:

- Full Time _

We are seeking a dedicated and compassionate Case Coordinator to join our team. As a Case Coordinator, you will be responsible for coordinating and managing patient cases, ensuring the delivery of high-quality care. This position requires knowledge and experience in pediatrics, coding, hospital procedures, behavioral health, and social work.

**Responsibilities**:

- Coordinate patient cases from admission to discharge, ensuring smooth transitions and continuity of care
- Collaborate with healthcare professionals to develop individualized care plans
- Conduct assessments and reassessments of patients' needs
- Provide support and guidance to patients and their families throughout the treatment process
- Assist with discharge planning and ensure appropriate follow-up care
- Maintain accurate and up-to-date documentation of patient information
- Adhere to HIPAA regulations to protect patient confidentiality

**Requirements**:

- MA Registered Nurse License
- Experience working in a hospital or healthcare setting
- Knowledge of ICD-9 coding and medical terminology
- Strong organizational skills with the ability to prioritize tasks effectively
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a multidisciplinary team
- Understanding of behavioral health principles and practices
