Acenda Integrated Health is recruiting for a Case Manager to support the Integrated Case Management Services Program in Cape May, New Jersey. _This position includes the option for a Hybrid work schedule._

Acenda's expert team helps each individual, family and community move forward in their journey towards health and happiness.

Key Responsibilities
- Engages consumers at the point of hospitalization and participates in the discharge planning process to assist with the transition to the community
- Provides post-discharge follow-up to support the consumer with community integration
- Conducts assessments within correctional facilities and attends court hearings to advocate for persons served

**Requirements**:

- Minimum requirement is Bachelor’s degree in psychology, social work or a related field, plus 2 years of relevant experience.
- Professionalism and a commitment to excellence in care
- Must have and maintain a valid driver's license with an acceptable driving record.
