**Job Description Virtual Rehab Care Coordinator**
Physical Medicine and Rehabilitation practice, specializing in post-acute rehab care, that is seeking a highly skilled and motivated Virtual Rehab Care coordinator to join our team. This is an excellent opportunity to be part of a dynamic healthcare practice, focused on providing exceptional patient care and promoting work/life balance for our providers.
**Key Responsibilities**:
To facilitate the process of case management. Duties include:
- Conduct initial comprehensive assessments for the purpose of the patient's care plan.
- Monitor and evaluate patient’s rehabilitation treatment plans and progress.
- Make appropriate and timely referrals, including documentation to post discharge providers/physicians.
- Facilitate referrals to outpatient care teams or Home Health Services for continued/additional community services as needed.
- Assist patients with timely procuring/planning of durable medical equipment including assistive devices.
- Monitor compliance with regulations for orthotics and/or prosthetics.
- Assist in the patient’s compliance with medication and care plan.
- Document patient encounters accurately and efficiently.
**Job Requirements**:
- 2-3 years of clinical experience (preferably in case management, Nursing, Social Work, or other rehabilitation/health related profession).
- Computer literacy including use of record-keeping software.
**Education & Training**:
- Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and/or Rehabilitation Counseling). **Medical Students welcome to apply**:
- If licensure is required for one's discipline within the state, the individual must hold an active license.
- CCM® or ACM certification preferred but not required.
**Job Types**: Part-time, Contract
Pay: $20.00 - $25.00 per hour
Expected hours: 10 - 15 per week
Schedule:
- Day shift
- Monday to Friday
Work setting:
- In-person
