Safeguard PHI by ensuring confidentially& ensuring patient records available only to authorized personnel, including assuring compliance with HIPAA regulations.
- Chart Completion
- Ensure PHI is scan and index into EMR of the appropriate and accurately patient electronic record (including loose files).
- Prepare patient chart for scanning and batching by verifying correct patient MRN, and correctly labeling documents.
- Utilize OnBase to scan, batch, and index medical records
- Review scanned documents for quality and accuracy.
- Ability to identify forms, standard and nonstandard, and their appropriateness for inclusion in the record.
- Master Patient Index (MPI)-Perform merges when appropriate after careful review using Allscripts, MediTech, and eCW.
- Timely retrieval of discharged charts from nursing units and ancillary departments. As well, as reconcile discharge records with daily ADT report, which includes tracking missing charts/documents and follow up.
- Update and reconcile scanned documents as needed for quality compliance.
- Use chart correction tools in Allscripts to move notes to different encounter, delete/move scanned documents, or move encounters from one patient record to a different patient record.
- Perform analyze medical record for completion of missing and/or unsigned documentation.
- Monitor the operative report system.
- Tracks undictated operative reports and follow up with appropriate physicians.
- Enter and delete deficiencies into chart deficiency system for tracking purposes.
- Notify Department Chairs of chart deficiencies
- ROI- Processes and prioritize requests for information from insurance companies, attorneys, patients, IPRO, Disability, physicians, federal and state agencies, multidisciplinary departments etc., as assigned.
- Opens and date stamps requests in a timely fashion, determines if requests are proper (i.e. valid authorization with a valid date, etc.); initiates a return letter if authorization is
inappropriate.
- Determines if exemption consent forms are necessary depending on the type of patient illness (i.e. psychiatric, HIV, etc.). Ensures that requests are processed accurately and in a timely fashion.
- Performs on-going follow-up of outstanding requests and informs the manager of request status when necessary.
- Responds to inquiries from patients and staff members and instructs them in the in appropriate methods for requesting records (includes phone calls). Forwards authorizations to appropriate areas.
- Enters requests accurately into ROI systems. Maintains accurate electronic files of requests and updates systems as needed. Scans authorization forms and related documentation into patient chart.
- Scans requested records into ROI systems and ensures that quality images are captured.
- Identifies missing or incomplete medical information and complies with departmental

procedures for following-up on missing or incomplete documents.
- Create digital or paper copies of requested information as needed. Distributes printed

information (or digital files) to authorized individuals. Appropriately grants access to electronic

records.
- Preparing invoices.
- Maintain and coordinate patient paper records stored in storage facility.
- Identify duplicate medical records and merge as required.
- Assist with departmental audits and investigations.
- Maintain quality and accurate records by following hospital procedures.
- Meet daily productivity performance measures.
- Performs other assigned duties as it pertains to HIM operations.
- Effective oral and written communication and interpersonal skills required.
- Must have knowledge of relevant computer systems and scanners.

Shift : Monday - Friday, 9:00 A.M. - 5: 00 P.M.

Schedule:

- 8 hour shift
- Monday to Friday
