Under direction of the Instructional Supervisor, the Home and Hospital Case Manager is responsible for the coordination and facilitation of the home instructional program in both face-to-face and virtual learning modalities for the district. The incumbent ensures educational services are provided for each qualifying student for the time-period designated by the student's physician or mental health professional or for students serving a long-term or extended suspension.

DUTIES & RESPONSIBILITIES: _**
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
- Assists with the coordination of instruction with other offices (school counseling, health services, 504 and psychological services) to ensure students are processed and served according to COMAR;
- Assists in the preparation and analysis of data reports;
- Consults with the director, supervisor and other staff members to establish program standards, to develop office goals, to develop a data monitoring system and to ensure effective program delivery;
- Maintains the administrative procedure, forms and pertinent programmatic documents;
- Maintains a database on student referrals to include fields for approvals, denials, start and end of instruction, assigned teacher, etc.;
- Oversees the daily operation of the office, staff-programming, payroll, student/home teacher assignments, and home and hospital teacher needs;
- Manages temporary hourly paid staff who render face-to-face instruction for home and hospital services or for alternative academic supports as well as those serving the Home Teaching Library;
- Provides written and oral communication related to verification, approvals, denials and service requirements;
- Responds to inquiries from students, parents, school personnel and legal to provide information, clarity and resolution to issues/complaints regarding student cases;
- Orients, trains, mentors and monitors new home and hospital teachers regarding procedures and delivery of instruction;
- Plans with content area teachers and school personnel in order to ensure appropriate instructional standards are established and implemented and that resources are made available;
- Facilitates training school staff members, community organizations and agencies, area hospitals, and parents/guardians regarding protocols and procedures for face-to-face or virtual home instruction;
- Processes incoming referrals and assigns a teacher to qualified students within 10 school days of receipt of referral docs as specified in COMAR;
- Gathers information needed for administrative decision making regarding student approvals;
- Serves as a consultant and partner by attending school-based meetings (SIT, SST, IEP, 504) as well as CIEP, compliance, court liaison, and inter-agency;
- Represents the supervisor and/or department at Maryland State Department of Education (MSDE), area hospitals, county agencies, and other community providers; and
- Performs other duties as assigned.

QUALIFICATIONS:_***
The requirements listed below are representative of the knowledge, skill, and/or ability required.
- Knowledge of federal and state laws and regulations relevant to HHT (e.g., Individuals with Disabilities Education Act; Section 504; MSDE Regulation COMAR 13A.10.01.01); and PGCPS Administrative Procedures regarding HHT;
- Considerable knowledge of methods, strategies and curriculum in one or more of the following areas: elementary school, middle school, high school, special education, and early childhood;
- Knowledge of curriculum, teaching techniques, and materials of instruction in field of specialization;
- Knowledge of school organization, goals and objectives, curriculum, teaching techniques, current trends, research, and materials of instruction in field of specialization;
- Excellent knowledge and understanding of the teaching and learning process;
- Flexibility, creativity, ability to adapt to a variety of teaching settings;
- Excellent time management, problem solving, conflict resolution and organizational skills;
- Ability to work effectively with students, parents, administrators, colleagues, community, and other school system staff;
- Ability to work with diverse populations;
- Ability to communicate clearly and effectively, both orally and in writing and to maintain effective working relationships; and

Bachelor's degree (Master's Degree preferred) in applicable field of education from a regionally accredited college or university and five (5) years of successful teaching experience. (Teacher leadership experience preferred)
