Job Title: Home Health Registered Nurse in Rutland,Vermont
We are seeking a compassionate and dedicated Home Health RN to join our team in Rutland, VT As a Home Health RN, you will be responsible for assessing patient/caregiver needs, developing and communicating a realistic plan of care, implementing medical plan of care, and coordinating a wide variety of patient care with physicians, providers, team members, vendors, etc. to ensure safe and effective patient and family care.
**Responsibilities**:
Utilize the nursing process to assess patient/caregiver needs and provide quality care to assigned patients in accordance with evidence-based practice, state and federal regulations, Vermont State Board of Nursing and VNAHSR policies and regulations.
Develop and communicate a realistic plan of care for each patient in accordance with evidence-based practice, state and federal regulations, Vermont State Board of Nursing and VNAHSR policies and regulations.
Serve as Case Manager for assigned patients and actively manage case loads and visit times in accordance with agency productivity standards.
Fill out and complete OASIS documentation in accordance with Agency policies and procedures, and state and federal regulations, demonstrating knowledge of, and keeping up-to-date on changes to OASIS documentation requirements as needed.
Implement medical plan of care, nursing interventions, and procedures as necessary to ensure the safe and effective quality of care for patients in accordance with evidence-based practice, state and federal regulations, Vermont State Board of Nursing and VNAHSR policies and regulations.
Stay up-to-date on a variety of High-Tech skills, such as IV use and maintenance, wound VAC, complex wound care, etc. to ensure infection control standards are maintained throughout plan of care.
Work collaboratively with LPNs to ensure that the plan of care is followed, delegate tasks, and ensure that patient needs are met.
Evaluate a variety of aspects of the implemented plan of care, including identified problems, care provided, and patients' responses, to meet patient/caregiver needs and ensure overall quality of care delivered.
Maintain up-to-date and accurate documentation of nursing care provided to ensure the effective integration of information for use by the health care team to ensure ongoing and continued quality of care.
Perform patient and caregiver education to ensure informed patient decisions can be made, including medication side effects, treatment options, discharge instructions, reportable conditions, ranges of services, etc.
Identify inter-disciplinary needs of the patient and document patient needs accordingly, including physical therapy, social work, etc. Take action to communicate documented information to other disciplines in a timely manner.
Accurately and completely fill out a variety of Agency, state and federal forms in a timely manner. Take action to ensure that forms are readily available during visits to facilitate timely completion.
Assess patients' needs for HCA services, develop and coordinate POC with patient/caregiver, and provide ongoing supervision in accordance with state and federal regulations.
Transmit patient incidents reports (infections, injuries, etc) to QI Specialist in a timely manner as necessary.
Coordinate safe patient discharges and/or transfers (or upon knowledge of) and ensure all appropriate paperwork and patient information is processed in accordance with department policies and procedures, and state and federal regulations.
Coordinate a wide variety of patient care with physicians, providers, team members, vendors, etc. in support of the recommended plan of care to ensure safe and effective patient and family care.
Participate in monthly peer review audits and provide constructive feedback and openly accept feedback in a professional manner.
Schedule, coordinate, and facilitate case conferences with Manager and team members to maximize effective communication with team members. Ensure effective and timely documentation of meetings is maintained.
Proactively advocate for patient care issues to ensure that overall quality
