**Blize Healthcare** is looking for a Registered Nurse or Licensed Vocational Nurse to join our team.

**Job Position**:

- Registered Nurse or Licensed Vocational Nurse (Full-time or Per Diem available).
- We're hiring in Solano County, Contra Costa County and Alameda County.
- Demonstrates the ability to make clinical judgments and utilize critical thinking in an effective and efficient manner with supervision.
- Perform according to nursing standards and is accountable for managing patient care and assisting others in the management of patient care.

**Required Skills & Qualifications**:

- Must have a minimum of 1 year of experience as an RN or LVN.
- Provide skilled nursing services/interventions in accordance with physician orders.
- Assesses patient needs plans for evaluation and modifies care to meet goals of skilled nursing interventions.
- Collaborates with all disciplines to plan and evaluate team goals for each patient.
- Experience in home health or hospice is recommended.

**Job Types**: Full-time and Part-time are available.
