As we continue to grow and expand our reach, we are seeking a dedicated individual to join our team as an Intake and Onboarding Coordinator.

**Purpose**

To support day-to-day operation of a growing non-profit organization.

**Status**

Full Time Non-exempt position, 40 hours per week.

**A typical day might include the following**:
Serve as the primary point of contact for participants referred to sonder SOLUTIONS for various services.

Manage the intake process for participants, ensuring all required referral documentation is collected and reviewed for completeness and accuracy.

Oversee the authorization process, ensuring that necessary approvals are obtained to provide services to participants.

Input participant information, referral documentation, and authorizations into the case management program, maintaining accurate and up-to-date records.

Coordinate with relevant departments and staff to facilitate the intake process and ensure smooth transitions for participants.

Provide onboarding support for participants entering paid work experiences, ensuring they have the necessary information and resources to succeed.

Collaborate with team members to continuously improve the intake and onboarding processes, streamlining procedures for efficiency and effectiveness.

**Essential Job Functions**

Participant Intake:
Serve as the primary point of contact for participants referred to sonder SOLUTIONS for various services.

Manage the intake process for participants, ensuring all required referral documentation is collected and reviewed for completeness and accuracy.

Oversee the authorization process, ensuring that necessary approvals are obtained to provide services to participants.

Input participant information, referral documentation, and authorizations into the case management program, maintaining accurate and up-to-date records.

Onboarding Support:
Provide onboarding support for participants entering paid work experiences, ensuring they have the necessary information and resources to succeed.

Coordination and Communication:
Coordinate with relevant departments and staff to facilitate the intake process and ensure smooth transitions for participants.

Collaborate with team members to continuously improve the intake and onboarding processes, streamlining procedures for efficiency and effectiveness.

Data Management and Reporting:
Maintain accurate and up-to-date records of participant information, referral documentation, and authorizations in the case management program.

Generate reports as needed to track intake and onboarding progress, referral sources, and participant outcomes.

Compliance and Confidentiality:
Ensure compliance with regulations, and organizational policies related to intake, onboarding, and data management.

Maintain confidentiality and handle sensitive participant information with discretion.

Guidance and Support:
Offer guidance and support to participants throughout the intake and onboarding process, addressing any questions or concerns they may have.

**Non-essential Job Functions**
- Following health and safety procedures that are consistent with State and Federal requirements. (Including passing needed safety certifications.)
- Participating in staff training issues in advance of team needs.
- Have the ability to be presentable in front of participants and business partners in a business environment.

**Requirements**:

- Must be 21 years of age or older.
- Must provide proof of identification and eligibility to work in the United States.
- Must pass a drug and background screening.
- Must possess and maintain a valid driver's license and auto insurance.
- Must have access to reliable transportation that does not interfere with performing the essential functions of the job description, cause tardiness, or unexcused absences.
- Two years experience in human services or in providing direct service in human service, educational, clinical services, case management or crisis services working with mental health, disability, homeless and/or at-risk youth.
- Associate's degree in human services, social work, or a related field preferred. Comparable experience will be considered.
- Strong organizational and planning skills, with the ability to manage multiple tasks and prioritize effectively.
- Excellent interpersonal, verbal, and written communication skills.
- Proficiency in Microsoft Office 365, including Outlo
