Housing Case Manager

The Rescue Mission is seeking a **full-time Case Manager** at **Whitney Place**, our supportive housing facility in **Binghamton, NY**. The Case Manager assists residents by assessing their needs and collaborating with appropriate services to aid residents in moving toward healthy independence. This position will also assist with maintaining a structured, safe, and supportive environment that reflects the mission, vision, and values of the Rescue Mission.

**Responsibilities and Duties**:

- Carries caseload and maintains timely and accurate documentation.
- Identifies strengths and barriers for each resident and completes case management assessments in a timely manner.
- Establishes and maintains rapport with each resident. Providing supportive counseling, encouraging and motivating residents toward positive growth.
- Assists residents with identifying attainable goals and works with them to attain those goals.
- Monitors progress toward desired outcomes through assessment and maintaining consistent contact with each resident.
- Reviews modifies, and follows up on resident assessments and action plans in a timely manner.
- Collaborate with the Regional Director regarding admissions/discharges. Coordinates alternative placement as necessary.
- Refers residents to appropriate internal and external service care providers.
- Networks with applicable individuals, agencies, and organizations within the community.
- Maintains client professional boundaries with clients as well as confidentiality by keeping a strict "need to know" policy and monitoring the handling of confidential information.
- Collaborates with community services involved with each resident; financial, mental health, addiction, legal, etc.
- Attends required meetings and events
- Regular and predictable in-person attendance is an essential requirement of the job

**Requirements**:

- Associate degree in human services or related field or equivalent work experience
- Bachelor’s Degree in social work or related field preferred
- Previous related work experience
- Bi-lingual skills a plus

**Pay**:
$18 per hour
