**Description**

The Lead Perinatal Care Coordinator provides training, leadership and guidance to Perinatal Care Coordinators, advise of workflow improvements or issues and support collaboration with partner organizations in addition to the core Perinatal Care Coordinator functions including being a resource and advocate for pregnant and post partum women, conducting maternal mental health screenings and systematically reviewing screening data, providing appropriate responses for identified needs around social determinants of health and collaborating with all involved teams and partners to ensure that patient and family needs are met including providing care coordination support for families experiencing difficulties getting needs met; ensuring optimal patient coordination and excellent collaboration between teams, with a focus on family experience and outcomes.

**Qualifications**

**Minimum Education**
Bachelor's Degree Bachelor's degree in social work, nursing, education, health sciences or a related field (required)

**Minimum Work Experience**
4 years Experience in a behavioral health or other health care setting as a member of a multidisciplinary team providing care coordination services to patients. (required)

2 years Experience working with pregnancy and/or postpartum patients, preferably providing care coordination services. (preferred)

**Required Skills/Knowledge**
Excellent communication skills required.

Ability to work with diverse patient populations

Ability to provide culturally appropriate trauma informed care and communication

Knowledge of resources available to meet the diverse needs of families in Washington DC

Excellent problem solving skills to support diverse program and patient needs

Excellent organizational and time management skills

Detail oriented, able to work independently in a fast paced collaborative environment

**Functional Accountabilities**
**Professional Practice/Research**
- Identifies patients/families appropriate for Care Coordination services in a systematic manner.
- Supports, implements and identifies resources for staff education regarding coordination of care, culturally sensitive care, compassion fatigue, and other relevant topics.
- Participates as part of a multi disciplinary team providing maternal mental health screening and response.
- Identifies and respond to care coordination needs of program patients
- Identifies and implement best practices for responding to identified patients needs

**Structural Empowerment**
- Follows through on any service needs or requests until complete and satisfactory resolution; updates the team as needed.
- Prioritizes patient needs.
- Serves as a resource for other team members in the area of community resources and care coordination workflows
- Supports linkages between patients, the health care system and social services resources.

**Exemplary Professional Practice**
- Completes documentation of all relevant conversations, meetings, and referrals.
- Actively communicates with team members and, if appropriate, escalate patent concerns or needs to the appropriate person or service.
- Actively participates in team meetings and raise priorities identified through working with patients and other staff.

**New Knowledge, Innovations, and Improvements**
- Supports the research related to identifying the best maternal mental health screening and response tools and protocols for optimal patient experience and outcomes.
- Provides regular updates to the program team regarding family support and patient status.

**Leadership**
- Updates team members as frequently as needed to ensure “one voice” consistent communication to families.
- Acts as a liaison among the DC Mother Baby Wellness Initiative team, medical providers and social service partners.
- Leads collaborative meetings with care coordinators.
- Provides relevant technical support and training to team members, collaborators and partner organizations.
