Established in 2023, CommuniCare+OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

**JOB SUMMARY** The Integrated Behavioral Health Clinician provides brief interventions and therapy integrated with other health providers, including Primary Care, Perinatal, Dental and other behavioral health and substance use providers. **EDUCATION, EXPERIENCE, TRAINING**
- AMFT/LMFT in good standing with the California Board of Behavioral Sciences.
- 1 year post licensed experience strongly preferred.
- Able to serve children under 12 years of age.
- Bilingual preferred; English/Spanish

**KNOWLEDGE OF WORK**

1.

Knowledge of medical terminology, DSM 5 and ICD-10 codes.

2.

Able to proficiently engage and provide care to patients by way of HIPAA complaint telehealth/video-based services and fully participate in video-based meetings and trainings

3.

Effectively utilize measurement-based care practices to determine client progress, wellness and risk factors; collect required departmental pre/mid/post health screenings (e.g., PHQ9).

4.

Identifies referral needs. If needed, makes referrals for more intensive psychological or psychiatric services.

5.

Ability to accurately and quickly attune with clients during Warm Hand Offs from Primary Care and Dental Providers.

**DUTIES AND RESPONSIBILITIES**

1.

Competently deliver therapy services to children and adults with client goals being at the center of care.

2.

Provide in-person and telehealth services including assessment, triage, crisis support, and individual and/or group therapy for patients experiencing a range of mental health symptoms in a trauma-informed and culturally responsive manner.

3.

Collaborate with primary care, dental, clinic staff, and Yolo County Health and Human Services Mental Health to provide and coordinate care, including substance use disorders and level of care changes.

4.

Independently manage complex caseload in back-to-back 30-minute windows and complete administrative duties in fast-paced medical clinic setting.

5.

Complete clinical documentation within 48 hours of services in accordance with working hours, all documentation must comply with Medi-Cal, Medicare, Partnership, Beacon Health guidelines. Participate in peer chart reviews and other quality improvement efforts.

6.

Available for in-person and video-based crisis, triage, and consultation during primary care and substance use disorder treatment hours.

7.

Complies with established department policies and procedures, objectives, quality assurance program, safety, environmental, and patient service standards.

8.

Based on the individualized patient assessment, develops a treatment plan of provision of services in collaboration with the client and includes measurable goals.

9.

Ability to address a wide range of mental health conditions, utilize a variety of assessment tools, facilitate diagnostic evaluations and treatment interventions to address client needs, goals, and circumstances.

10.

Refers patients with apparent need for psychotropic medication to primary care provider and/or psychiatrist where appropriate.

11.

Participates in integrated system of care, staff meetings, case review, consultation/supervision sessions and other forums as required.

12.

Conducts all work within generally accepted standards and ethical guidelines of the social work or counseling professions. Seeks out guidance from Assoc
