**POSITION TITLE**: Medical Records Clerk

**REPORTS TO (TITLE)**:Health Information Management Manager

**POSITION STATUS**: Non-Exempt

**DESCRIPTION OF POSITION**:
The Medical Records Clerk is responsible for coordinating and guaranteeing accurate scanning and filing of medical records, ensuring medical records are complete and there is proper release and maintenance of records. The Medical Records Clerk of Anaheim Community Hospital (ACH) has the responsibility to comply with state and federal regulations and to comply with current HIPPA regulations. This position will require proficient use of our EHR (Electronic Health Records) system. Other aspects of this role will include communication with Physicians and other medical staff to ensure accuracy of medical records, respond to requests for medical records, answering phones, and assisting the Health Information Manager as required.

**KEY RESPONSIBILITIES**:

- Monitor medical records for timeliness, completeness, and accuracy by following hospital policy.,
- Collect patient records from various units.
- Maintain and assemble patient records into standard order.
- Files records in designated areas or electronic medical records system.
- Analyzes discharged records for required reports and signatures in a timely manner.
- Accepts modeming via computer and routes transcription accurately and in a timely manner.
- Ensure patient charts, paperwork, and reports are completed in an accurate and timely manner.
- Accurately files loose reports in medical records in their proper places.
- Efficiently and appropriately processes requests for information from authorized requestors.
- Proficient navigating and analyzing using EMR.
- Scans loose filing daily and e-files accurately adhering to EMR categories and document types.
- Keep all medical records confidential and protected.
- Review daily admits and discharges for accuracy.
- Supply the nursing department with the appropriate documents and forms
- Perform any other functions as required by management.

**Minimum Requirements**:
Education and Licensing:

- High school graduate or GED.

Knowledge and Experience:

- A minimum of 2 years' experience in a similar role.
- Understanding of medical terminology and administration processes, preferred.
- Relevant training and/or certifications as a Medical Records Clerk, preferred.
- Proficient using computers for word processing including MS Office, database management, and information management programs.

Skills and Abilities:

- Assists physicians and staff, as necessary, in a courteous and efficient manner.
- Processes mail according to approved protocol.
- Handles all requests by the public in a professional manner, whether via phone or in person.
- Strong attention to detail with excellent organizational skills.
- Outstanding communication and interpersonal abilities.

**Physical Requirements**:
While performing the duties of this job, this position is frequently required to do the following:

- Use standard office equipment and access, input, and retrieve information from a computer.
- Use computer keyboard with manual and finger dexterity and wrist-finger speed sufficient to perform repetitive actions efficiently for extended periods of time.
- Communicate effectively in person or via telephone in a manner which can be understood by those with whom the person is speaking, including a diverse population.
- Give and follow verbal and written instructions with attention to detail and accuracy.
- Perform complex mental functions and basic arithmetic functions; interpret complex laws, regulations, and policies; collect, interpret, and/or analyze complex data and information.
- Vision: see details of objects at close range.
- Coordinate multiple tasks simultaneously.
- Reach forward, up, down, and to the side.
- Sit or stand for minimum periods of one hour at a time and come and go from the work area repeatedly throughout the day.
- Lift up to twenty-five (25) pounds.
