**Duties**:
**This Registered Nurse position is located in the Patient Aligned Care Team (PACT), Ambulatory Care Service at the Manchester VA Campus location. This position is full-time at 40 hours per week.**

**Included but are not limited to**:

- Plays an integral role in the Patient Aligned Care Team (PACT) providing initial and ongoing assessment of patients and functioning as a systems coordinator for the plan of care.
- Initiates PACT pre-planning for scheduled clinic visits, participates on daily huddle and team meetings to collaborate how best to meet the patients' needs.
- Conducts follow up calls to hospitalizations and Emergency Department visits, reviews records, and coordinates follow up care.
- Participates in direct patient care surrounding scheduled and unscheduled visits.
- Manages a designated panel of patients and utilizes VA developed protocols to enhance preventions as well as manage chronic diseases including HTN, DM, COPD, CHF
- Identifies and care manages high risk patients via face-to-face visits, virtual video connects, secure messaging and telephone calls.
- Coordinates transitions for patient care with specialists and consultants
- Conducts comprehensive assessment of patients' needs and ensures patients are linked with all appropriate services, resources and opportunities based on his or her needs.
- Partners with Veteran to support self-management, prevention, and wellness, utilizing Health Coaching and Motivational Interviewing to assist with behavior change and guidance towards his or her goals.
- Provides patient-centered, evidence-based, patient driven care in all patient interactions across the care continuum.

**Qualifications**:
**Basic Requirements**:

- **English Language Proficiency. **In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
- **Graduate of a school of professional nursing** approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); **OR** Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program.
